Payment Options
We set our fees in accordance with standard charges for the services rendered. Moreover, we participate with most health insurance companies, and accept their fee schedule for covered services. Please call our offices to be sure that we do accept your insurance. We routinely submit claims, as a courtesy to our patients, to those insurance companies with whom we participate. Payments are due at the time of service and may be made by cash, check, Visa or MasterCard. If you wish to know the fee for a visit or particular treatment, please ask and our billing manager will be glad to provide you with this information. Any balance not covered by insurance, such as co-payments or deductible amounts, or if we are not a participating provider in your insurance plan, will be your responsibility and is due at the time of service. Auto accident injuries and Worker's Compensation cases may be covered but must be confirmed on an individual basis. Please note that insurance coverage is subject to change, and we understand that medical bills and insurance claims are expensive and often confusing. It is not possible for us to keep track of the wide range of coverage and continuous changes related to all of our patients' insurance plans. Insurance coverage may vary even within the same insurance company, depending upon the specific plan chosen by the insured. If you have any questions, please feel free to contact our billing manager.

Appointments
We see all of our patients on an appointment basis and ask that you call in advance so that we may reserve a time for you. We try to see all patients on time and expect that you call and notify us at least 24 hours in advance if you cannot keep a scheduled appointment.

Please bring a list of the medications and dosages that you are taking to your appointment.

It is important that you call your primary care physician's office prior to your appointment to make sure that the necessary referral has been made.

Provide us with your health insurance information prior to your appointment.

Insurance
Our office accepts a variety of HMOs, PPOs, and other health plans. Please call our office to verify acceptance of your insurance carrier.
